“The mission of Valley High School Sports Medicine is to provide optimal professional healthcare to student-athletes by way of prevention, immediate care, and rehabilitation of athletic injuries. The student-athletes of Valley High School should feel that their overall health and well-being is the number one priority of the sports medicine program”
APS Sports Physical
Beginning Monday, August 22, 2022, no paper copies of physicals will be accepted. All sports physicals must be submitted electronically to DragonFly. Please contact Mr. Paz if you have any issues with this.
Per NMAA Bylaw 6.15, the New Mexico Activities Association sports physical packet must be used for all preparticipation examinations and all forms must be submitted to your school prior to participation. Sports physicals must be completed on or after April 1, 2023, to be valid for the 2023-2024 school year.
Starting in the 2023-2024 school year all sports physical forms will be available electronically in DragonFly to fill out and complete. The Physical Examination and Medical Eligibility Form will be the only forms that need to be uploaded to DragonFly and can be found below.
Help to create an account for Student Athletes and Parents can be found below. It is highly recommended that the parent makes an account and adds their child to their account.

Letter To Parents
Athletes that are referred or choose to see a physician on their own must bring a note from the physician with a diagnosis and what you can/ cannot do. Athletes going to see a physician must download and print the physician evaluation form below to have your physician complete. Athletes that have a skin condition must download and have your physician complete the skin condition release form.
