Physical Packets - Turn In to The Athletic Trainer

PHYSICALS - The New Mexico Activities Association (NMAA) and Albuquerque Public Schools (APS) require sports physicals to be completed on or after April 1, 2021 to be valid for the 2021-2022 school year. In the past, Valley High School (VHS) allowed student athletes to use these physicals to participate in summer workouts. In the interest of student athlete safety, scientific evidence, and best practice VHS has changed this policy. Student athletes are now required to have a current physical (dated on or after April 1, 2021) to participate in summer athletics.

Completed NMAA Physical Packets must be turned in to the athletic trainer directly. Packets can be downloaded and printed or picked up from the athletic training room or the Valley High School Front Office. Summer AT room hours are posted below and summer front office hours are Monday-Friday 7am-3:30pm.

These packets may be submitted to the athletic trainer via email (Tiffany.Shelton@aps.edu) or in person appointment. In person meetings can be scheduled using the marron button to the right. You must submit the correct physical form fully completed or you will not be cleared and will need to resubmit or schedule another appointment. Please allow at least one full business day for response to all physical packets submitted via email. 

APS Is Offering Physicals: The School Based Health Clinic at Albuquerque High School can do sports physicals, usually on the same day that the student calls.  They take most insurances, or charge $25 dollars if someone does not have or does not want to use insurance.

Athletic Forms - Turn in to your coach

The Athletic & Activity Code, APS Consent to Participate, and Concussion Management Course documents must be turned in to your coach. You will also need to complete the Athletic Information Sheet. Responses will automatically be sent to the athletic director.

On November 7, the NCAA Eligibility Center hosted a webinar for high school student-athletes and their families, focusing on navigating initial eligibility during COVID-19.
